Peculiarities ofpathogenesis and treatmentof dyspepsia occurs in childrenwith atopic dermatitis
Abstract
In the article there is presented data, which demonstrates the peculiarities of pathogenesis of the dyspepsia syndrome among children with atopic dermatitis. In particular, the higher acidity level was revealed in this group in comparison with children without atopia. According to the data of pH-measuring the acidic profile of children with atopic dermatitis is approaching to the same of the ulcer disease, in particular, in both cases most of the 24-hour period pH level does not exceed 1,9 (respectively 56% and 60%). The obtained results allow to recommend more wide usage of antisecretory remedies in therapy of dyspepsia syndrome among children with atopic dermatitis.

Life qualityand adherenceto therapy of old patients withchronic cardiacinsufficiency
Abstract
To assess the quality of life and compliance, 48 elderly patients (23 males and 25 females)
with chronic heart failure were examined. All patients received a standard treatment,
including ACE-inhibitors, ?-blockers, diuretics, selective aldosteron receptor antagonists
and digoxin. In addition, 24 patients were individually educated and received psychological
advising. We estimated the dynamic of functional class of CHF during the treatment and
compliance in all patients.
Initially 65% of patients had increased level of anxiety and reduced quality of life. After
complex treatment psychological status and quality of life were certainly better in all
patients, especially in main group, which received educational programs. At the beginning
of treatment compliance was low, but effective medical consulting helped patients to follow
medical advice.

Arterial hypertensionand carotic arteriaatherosclerosis: diagnosticpeculiarities, course andprinciples of surgical correction
Abstract
Arterial hypertension with the stenosis of carotid bifurcation is hardly corrected by
medicines and traditional anti-hypertensive therapy is ineffective.
The aim of our investigation is to reveal the relationship between arterial hypertension
and hemodynamically important stenosis of carotid bifurcation, to study the course of
arterial hypertension before and after the operation of carotid endarterectomia. The
investigation is performed on bases of chair of inner diseases and facultate surgery of Samara
state medical university. It includes 176 patients at the age of 39-69 years old. All the patients
are divided into 3 main groups: 1st - the control group (52 patients); 2nd(124)- patients
with the hemodynamically important stenosis of carotid bifurcation before the operation
of carotid endarterectomia; 3rd (124)- the same patients after the operation of carotid
endarterectomia. All the patients are performed 24-hours monitoring of blood pressure and
the additional instrumental investigations to reveal the possible reasons of the secondary
arterial hypertension. After our investigation we revealed that the level of blood pressure
after the operation of carotid endarterectomia became lower than before the operation.
We study this phenomenon and we insist in calling this type of arterial hypertension - the
secondary cerebrovascular arterial hypertension. We describe three types of the secondary
cerebrovascular arterial hypertension. After our investigation also we recommend to
perform the operation of carotid endarterectomia by the classical method without resection
of carotid glomus nerve in cases of the second-side operation of the same patient.

Purpose. To study the influence of the statins on the lipid exchange, hemostasis and
endothelial function in patients with ischemic heart desesase. Methods. For the treatment
all the patients were divided into the categories of high and very high risk. In this patients
the lipid exchange and hemostasis are studied. Results. Statin therapy reliably reduces the
total cholesterol, LDLCH and triglyceride levels. Statin therapy also reliably reduces the level
of von Willebrand factor and the trombocyte aggregation activity.

Life quality andpsychosomatic portrait of countrypeople living in Samara region sufferfrom arterial hypertension
Abstract
In our investigation we studied the life quality of 773 patients with arterial hypertension
of Samara region using questionnaire SF-36, including 390 countrymen (52,5 ± 11,9 years
old) and patients of control group before and after the educational program of arterial
hypertension. Using LOBI test, scale by Spielberg, MMSE, we defined psychosomatic
portrait of patients with arterial hypertension of countrymen and Railway hospital on
Samara station. In our work it was revealed that life quality of countrymen is low (p<0,05),
because of the psychological part - decrease of cognitive function, anxious and neurasthenic
relationship to disease, high personal and situational alarm at the base of low compliance.
Therapeutic prevention schools of arterial hypertension raises life quality of examined
groups.

Estimation ofefficiency of pharmacotherapystandards introduction for bronchialasthma, hypertonic disease and ulcerdiseases at Autovaz
Abstract
The objective of the research is an assessment of the efficiency of the optimization of
pharmacotherapy by means of pharmaco - epidemiological and pharmaco - economical
analysis at the JSC «AVTOVAZ». The work was carried out since 2006. The total amount of patients,
morbidity rates and work incapacities that were studied made 16 thousand people in 2006 and 17
thousand people in 2007. The issues of interference were the follows: the number of aggravations
per year, the quantity of calls of first aid and average duration of temporal work incapacity. The
implementation of pharmacotherapy standards of bronchial asthma and hypertensive disease
in the first year did not led to the statistically significant decrease of the quantity of aggravations,
but the number of first aid calls had decreased, and also the average duration of temporal
work incapacity with the cause of hypertensive crisis had decreased. The implementation of
pharmacotherapy standards of ulcer had led to statistically significant number of aggravations
and to the decrease of average duration of temporary work incapacity.

Urinary bladder cancerin Saransk
Abstract
The morbidity of a urinary bladder cancer in Saransk in 1.7 times above, than on the
average across the Russian Federation. Thus, from 1996 to 2008 the quantity of urinary bladder
cancer has increased in 2.8 times. Men fall ill urinary bladder cancer on the average in 4 times
more often. The peak of morbidity at men is necessary on age 65 - 69 years, at women - 70
- 74 years. The share of patients with I - II a stage of urinary bladder cancer has increased by
19.2 %, at decrease on 10.9 % of number of patients with IV stage of tumor process. Death rate
from urinary bladder cancer has decreased with 5.0 to 3.1 on 100 thousands the population
at decrease one-year lethality for 26.1 %.

Results of surgicaltreatment of colorectalmetastasis inlungs with application of radiofrequent ablation methodand atypical resection
Abstract
We led out the comparative analysis of treatments results in two groups of patients with
diagnosis colorectal cancer after surgical treatment with progressive process: single Mts in
lungs. There were made lungs atypical resections in the 1 group (42 patients), in the 2 group
(27 patients) - RFA of lungs Mts. The greatest percent of postoperative complications was in
the 1 group (9,5%), but this number was only 3,7% (р=0,64) after RFA. Two-years survival of
patients with colorectal Mts in lungs after lungs atypical resections was 64%, but after RFA
- 71% (р=0,17). The research of lifes quality in both groups of patients by Karnovskys scale
and ECOG-WHO scale reveal the next: through 1 year after operation these indexes were 73%
and 2,2 points accordingly in the 1 group, but in the 2 group - 79% and 1,6 points (р<0,05).
The next estimation of lifes quality was led through 2 years: lifes quality was estimated in
65% and 2,7 points accordingly, but in the 2 group - 74% and 1,7 points (р<0,05). Conclusions.
RFA of lungs Mts from colorectal cancer is a lowdamage, organs presses method with less
number of post operative complications. The indexes of two-years survival and lifes quality
of patients trough 1 and 2 years is better after RFA than the same indexes in the group of
patients after lungs atypical resections.

Mexidol influenceon antimetastic activityof anticancer drugs
Abstract
The present study aimed to investigate the influence of combination of the synthetic
antioxidant mexidolum (3-oxy-6-metyl-2-etyl-piridin succinate) and antitumor drugs
- cyclophosphamide, 5-fluorouracil and rubomycine on cancer metastatic process. Studies
were performed on C57Bl mice which B16 melanoma had been implanted s.c. The volume
of primary tumor, quantity of lung surface metastases, ability to inhibit metastases were
determined. Mexidolum at a dose of 10 or 50 mg/kg body weight was injected i.m. daily
followed by tumor transplant for 25 days. Results showed that mexidolum significantly
increased antimetastatitic efficacy studied antitumor drugs.

Abstract
Investigation of risk factors of lung cancer is still actual world wide. Definition of
connection of all possible risk factors within etiogenesis of lung cancer is still not possible.
Thus case histories of 876 patients with lung cancer were analyzed. The geographical
dissimilar distribution of the patients within regions of Republic was revealed. The cases of
citizens were analyzed. It was determine that lung cancer affects more often men, people
over 50 and right lung within citizens. Majority of the patients besides smoking have taken
alcoholics. In most of the patients, who live in the city, the late stages of lung cancer and
central form of anatomical growth of the tumor are diagnosticating.

Diagnostic possibilities of osteocyntigraphia applicationwith combination of chronic posttraumatic osteomyelitis with bony nonunion
Abstract
The article discusses the questions of chronic posttraumatic osteomyelitis combined with bone nonunion. The treatment results of such patients depend on the correctly chosen therapeutic management. When choosing the therapeutic approach surgeons are also guided by the revealed tendency of reparative osteogenesis in the nonunion zone. Diagnostic opportunities of osteoscintigraphy for such cases are not adequately explored. The monitoring results of bone tissue reparative regeneration in the nonunion zone of 72 patients are compared in this research, with or without using osteoscintigraphy. Scintigram data handling and interpreting are carried out by author's strategy. The results obtained show great diagnostic value of osteoscintigraphy. Carried out investigations demonstrate that using scintigraphy promotes making a sound tactical decision at an earlier stage.

Analysis of surgicaltreatment results of acuteappendicitis from mini-approach
Abstract
It is developed and introduced slanting pararectal miniaccess to a dome of a blind gut

are revealed. Duration of operation appendectomy from miniaccess averaged 39,5 minutes,

Restorative surgery of auricle:history and perspectives
Abstract
Auricles are the one of the most important anatomic formations of maxillofacial
region. Therefore, their defect or full absence promote the development of an inferiority
complex in the patient, and influences his or her social life negatively. Till now the most
known multisteps method of ear reconstruction with use autogenous rib cartilage wich was
known in 60-70 years. For patients with total and subtotal defects of auricles, as a framework,
we use individually simulated silicone implant which allows us to reproduce as much as
possible precisely parameters of an auricle, its relief and the corner of assignment which
is the necessary condition for creation post auricle fold. As a covering for implant we use
temporoparietal fascial flap which is a high-grade plastic and biological covering and also
a favorable basis for engraftment of splintered autograft skin transplantat. The using of an
auricle silicone implant as a framework, is necessary because of its elasticity, durability,
inertness and biocompatibility. Thats why we keep received result, and allows the patient to
sleep on the generated auricle, not affecting its blood supply.

The speech production study in mild depressed patients is an actual trend of clinical
psycho-pathology development in the aspects of diagnostic significance. This article
presents the study of the superficial speech structure organization among patients suffered
from neurotic depressions. The features of the superficial speech structure organization in
lexical level, grammar and syntax and their correlations with the clinical picture specifics
and psychological features are revealed. The research concluded that psycholinguistic study

research. Its been first time determined that whole body vibration exercises stimulating
proprioreceptors of skeletal muscles cause significant airway smooth muscles relaxation
resulting in increased maximal expiratory flow in men of various ages. Moreover, the
increase in MEF50 and MEF75 parameters was twofold higher in elderly as compared to
young. These findings suggest that acute WBV exercises may be beneficial and valid treating
and preventive technique for smart decrease of airway smooth muscle tone in patients
suffering from bronchial obstruction.

Abstract
The experience of management optimization in contemporary medical organization is
described in the article above taking an example of medical-prophylaxis service of one of the
biggest industrial facilities called AVTOVAZ JSC on the basis of the process management.
Process management is based on structural and functional modeling of both medical services
(including clinical-diagnostic processes, prevention, rehabilitation) as well as management
technologies (planning, control, finance flows management, information management, HR
management).
The complex of management technologies is based on decision making improvement
to be able to achieve the Major Objective - improvement of the health status of facility
manpower.

Social efficiency of One day surgerydepartmentin multi-profilemedical institution
Abstract
One of the basic criteria of the estimation activity of TPI is social efficiency. On the example
of One day surgery department, created in 1998 on the basis of MSU №5, the author carried
out complex research of social efficiency of the given department. The socially-demographic
medical services portrait of the consumer is established. Satisfaction of patients is defined
by quality of medical aid while staying in the department. The patients opinion on state of
health in the postoperative period is studied. On the basis of the received data the author
comes to the conclusion that 84,5 % of respondents prefer surgical treatment in One day
surgery department

Modern educationalprocess and physicaldevelopment ofschoolchildren in Samara
Abstract
In the article is represented the analysis of the physical development and health condition
of 1097 pupils from 1st to 11th grade, who study in three educational establishments:
general school, lyceum and gymnasium. It was found out, that closer to finishing the school
the amount of children with faults in posture increases more than up to 7 times, with flatfootedness
up to 3-4 times, and with eyesight malfunction is twice as much. Maximum
performance of the malfunctions is marked in the educational establishments of the new
type. The majority of the students are related to the second health group, during the study
process the quantity of children with first health group decreases and with the third group
increases, what tells about the decreasing health level of pupils.
