多螺旋计算机断层扫描彩色图谱与磁共振成像在诊断儿童急性血源性骨髓炎髓内阶段中的结果对比分析

详细
论证。尽管急性血源性骨髓炎被认为是一种研究相当深入的疾病,但诊断错误的发生率仍然相当高。由于症状、体征和实验室检查结果的多变性和非特异性,骨髓炎很难被发现。早期诊断对疾病的成功结局至关重要,因为治疗不及时会增加并发症的数量。
目的。本研究旨在评估磁共振成像和多螺旋计算机断层扫描方法在诊断急性血源性骨髓炎髓内期最早征兆方面的信息量。
材料与方法。30名疑似急性血源性骨髓炎患者接受了磁共振成像和多螺旋计算机断层扫描,并进行了彩色绘图和X射线密度评估。在诊断算法的最后阶段,还进行了骨密度测量。同时,采集骨髓管内容物进行微生物学和细菌学研究。
结果。在急性血源性骨髓炎的髓内阶段,96%的病例通过磁共振成像和多螺旋计算机断层扫描发现骨髓水肿迹象。我们注意到,磁共振成像的灵敏度为96%,多螺旋计算机断层扫描的灵敏度相同,但特异性明显低于多螺旋计算机断层扫描彩色图谱和放射学密度评估,分别为67%和83%(P<0.05)。
讨论。近年来,计算机断层扫描在诊断儿童急性血源性骨髓炎中的作用已在儿科外科实践中得到相当程度的认可,而在诊断儿童急性血源性骨髓炎中使用多螺旋计算机断层扫描并结合彩色图谱和放射学密度评估的应用则相对较晚。与此同时,许多研究人员都报道了多螺旋计算机断层扫描在诊断急性血源性骨髓炎中的高信息价值。
结论。1.磁共振成像和多螺旋计算机断层扫描显示,急性血源性骨髓炎髓内期的最早表现为骨髓水肿。
2.根据本研究,多螺旋计算机断层扫描彩色图谱和X射线密度估算具有较高的特异性,可与磁共振成像共同作为诊断急性血源性骨髓炎髓内期的主要方法。
